1) Description of the subject matter of the contract (OPZ) constitutes an appendix to the Terms of Reference (hereinafter referred to as SWZ).2) Each Economic Operator may submit a Tender for one or more parts of the contract, however it should be emphasized that for each of the parts he is obliged to indicate a different person to be implemented. This is due to the fact that one person can act as an expert only in one of the areas listed in the parts. This means that regardless of the scope of qualifications held by the person assigned to carry out the contract, this person can only provide expert services in one of the seven areas. The Contracting Authority also does not allow the Economic Operator to indicate one person as part of the service provided by two experts (two persons) under parts 1,2,3. In the offer form - in case of submitting offers for more than one part - in each of the parts of the contract a different person shall be indicated, however for parts 1,2,3 the Contracting Authority requires indication of two experts (two persons) for each of these parts of the contract. Should the Contracting Authority find out that the same person has been designated to provide services for minimum two parts of the contract - the Economic Operator's offer for all parts shall be rejected. Indication of one expert (one person) to execute the contract within the framework of parts 1,2,3 also results in rejection of the offer.3 The contract is realized within the framework of the project entitled "Health Needs Maps - Database for System and Implementation Analyses", co-financed by the European Union from the funds of the European Social Fund under the Operational Program Knowledge Education Development.
