Description

The subject of the contract is the implementation of the evaluation study entitled: "Comprehensive evaluation of the Integrated Territorial Investments instrument under the framework of the Polish Republic of Poland 2014-2020". In order to confirm that there are no grounds for excluding the Economic Operator from participation in the procedure specified in Chapter VIII of the ToR, the Economic Operator (whose tender was the highest rated) shall be obliged to submit: 1) certificate of the competent head of the tax office confirming that the Economic Operator is not in arrears with payment of taxes, issued not earlier than 3 months before the deadline for submission of tenders or requests to participate, or any other document confirming that the Economic Operator has concluded an agreement with the competent tax authority on payment of these receivables together with possible interest or fines, and in particular has obtained an exemption, deferral or division into instalments of overdue payments or suspension of the whole execution of the decision of the competent authority; 2) certificate of a competent local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or any other document confirming that the Economic Operator is not in arrears with payment of social or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders or requests to participate, or any other document confirming that the Economic Operator has concluded an agreement with the competent authority on payment of these amounts due with possible interest or fines, in particular has obtained an exemption, deferral or division into instalments of overdue payments or suspension of the whole execution of the decision of the competent authority; 3) declaration of the Economic Operator on not being in arrears with payment of local taxes and charges referred to in the Act of 12 January 1991. on local taxes and fees (Journal of Laws of 2019, item 1170, as amended) - the form (template) of this Statement is attached as Attachment No. 7 to the ToR);4) information from the National Criminal Register to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the PPL Act, issued not earlier than 6 months before the deadline for submission of tenders;5) statement of the Economic Operator on the lack of a final court judgment or a final administrative decision on arrears in payment of taxes, fees or social or health insurance contributions or - in the case of such a judgment or decision - documents confirming payment of these receivables together with interest or fines, if any, or conclusion of a binding agreement on payment of these receivables (form - specimen - this statement is attached as Appendix 8 to the ToR);6) declaration of the Economic Operator on the absence of a ruling against him as a precautionary measure of the ban on tendering for public contracts (the form - model - of this declaration constitutes Annex 9 to the ToR). If an Economic Operator has a seat or place of residence outside the territory of the Republic of Poland, instead of documents referred to in item 13:1) point 4 - he shall submit information from a relevant register or, in the absence of such a register, another equivalent document issued by a competent judicial or administrative body of the country where the Economic Operator has a seat or place of residence of the person to whom the information or document relates, within the scope defined in art. 24 item 4. 1 points 13, 14 and 21 of the Act on the Public Procurement Law;2) points 1-2 - he shall submit a document or documents issued in the country where the Economic Operator has its seat or place of residence, confirming respectively that: a) he is not in arrears with payment of taxes, fees, social or health insurance contributions or that he has concluded an agreement with a competent authority on payment of these amounts together with possible interest or fines, in particular he has obtained an exemption, deferral or division into instalments of overdue payments or suspension in full of execution of the decision of a competent authority.
