Description

The subject matter of the contract is the maintenance of the building of the Centre for Biological and Chemical Sciences (Stages I and II) together with equipment, installations and fittings.2 Detailed description of the subject matter of the contract (hereinafter also referred to as OPZ) is attached as Annex 1 to the Terms of Reference, hereinafter referred to as the Specification or ToR.3The Contractor must perform the contract in accordance with all conditions specified in the ToR, including the "Detailed description of the subject matter of the contract" attached to the Specification and in accordance with the model contract (Chapter III of the ToR).4 Pursuant to Article 29(3a) of the Act, the Contracting Authority shall require employment under a contract of employment, by the Contractor or subcontractor, within the meaning of the Act of 26 June 1974. - Labour Code (Journal of Laws 2018, item 917, as amended), all persons performing activities in the course of the contract specified in the OPZ (Annex 1 to the ToR) - coordinator and maintainers.5.No later than on the day the agreement comes into force, the Economic Operator shall provide:1) a personal list of employees assigned to the execution of the order with information on the basis for disposal of these employees, which shall constitute an annex to the agreement; 2) a statement of the Economic Operator or the subcontractor on the employment of persons performing activities under the employment agreement. The statement should include in particular: the exact identification of the entity submitting the statement, the date of submission of the statement, indication that the activities covered by the summons are performed by persons employed under an employment contract together with the number of such persons, their names and surnames, type of employment contract and the size of the job and the signature of the person authorized to submit the statement on behalf of the Contractor or sub-contractor.The provisions of art. 46 sec. 5 item 3 of the Act shall apply accordingly.7The Contracting Authority does not allow for the possibility of submitting partial bids.8 The Contracting Authority does not allow for the possibility of submitting variant bids.9 It is recommended that a local inspection be carried out at the place of execution of the subject matter of the contract in order to obtain information for the correct and complete preparation of a bid.10 The date of the local inspection should be agreed by telephone with the Contracting Authority: Mr. Piotr Kostanek tel.22 55-26-563 from Monday to Friday during the hours: 9.00 - 15.00.00.During local inspections we do not provide any information concerning the description of the subject of the contract.
