The Federal Bureau of Prisons (Bureau), Community Treatment Services Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health services in Phoenix, Arizona. The services provided should be within a 5 mile radius of the Phoenix City Hall, 200 W. Washington St., Phoenix, AZ 85003 and within a half mile walk to a bus stop. Performance periods under this contract will be for a one-year base period to begin on December 01, 2019 with four (4) one-year option renewal periods with service ending on November 30, 2025 (if all options are renewed). The following required services may be sub-contracted to other licensed contractors.

Substance Abuse Treatment Services:

Substance Use Intake: (one per authorize inmate) for 60 inmates = 60 intakes.

Individual Counseling Sessions: 48 units (30 minutes = 1 unit) x 60 (per inmate) = 2880units

The services shall be in accordance with the Federal Bureau of Prisons Community Treatment Services most recent Statement of Work dated September 2017, PSB.
