Description

The subject of the contract is the supply of medicinal products, whose detailed description, assortment and estimated quantity is contained in the Assortment and Price Form constituting Annex 5 to SWZ.2 Deliveries shall be made successively in terms of quantity and assortment according to the Ordering Party's partial orders within a maximum of 2 days from placing an order, submitted by fax, phone or e-mail by the Ordering Party.
The subject of the contract is the supply of medicinal products, whose detailed description, range and estimated quantities are contained in the Assortment and Price Form constituting Annex 5 to the SWZ. 2. supplies will be made successively in terms of quantity and range in accordance with partial orders of the Purchaser within a maximum of 2 days from placing an order, submitted by fax, telephone or e-mail by the Purchaser. The Contractor shall ensure supplies of medicinal products in exceptional situations in the so-called 'on call' mode, i.e. not later than within 6 hours from the order on any day, including Saturdays and public holidays.4 The ordered assortment shall be delivered by the Contractor at his own expense to the Purchaser's registered office.5 The Purchaser points out that the quantities specified in the Assortment and Price Form are estimated demand for the duration of the contract. The Ordering Party reserves the right to use incomplete quantities of the assortment. The Ordering Party shall use 80% of the quantity of the products specified in the task. The remaining 20% of the quantity shall be used if necessary. The Contractor shall not be entitled to compensation claims against the Ordering Party for not using the full amount of the assortment. 6. The products must comply with all conditions arising from the law in Poland, which also applies to the listed medical devices. If it is impossible to supply the goods due to discontinuation of production of a given product during the term of the contract and there are no substitutes on the market, the Contractor shall present current documents confirming this fact. The documents must be issued by the manufacturer.8. The Contracting Authority shall require that the medicinal products offered are admitted to marketing in the territory of the Republic of Poland.9.
