The subject of the tender is selection of a contractor who will provide the Ordering Party with supplies based on successive supply of pharmaceuticals (medicines) by the contractor to the Dr Tytus Chałubiński County Hospital in Zakopane, 10 Kamieniec Street, 34-500 Zakopane, divided into 12 (twelve) packages according to the specification presented in point 4.10 of the SWZ.2.The Ordering Party stipulates that the supplied items must have the required permits, certificates and attestations as required by current legal regulations, e.g. permits, certificates, attestations proving that they have the required permission to be marketed and used in Poland according to their intended use.3. The subject of the contract must be marked by the manufacturer in such a way that it is possible to identify both the product and the manufacturer.4. The Ordering Party stipulates that the execution of supplies will take place gradually based on partial orders submitted by the Pharmacy Manager using electronic communication means.
The subject of this part of the contract is selection of a contractor who will provide the Ordering Party with deliveries consisting of successive supplies of pharmaceuticals (medicines) by the contractor to the Dr Tytus Chałubiński County Hospital in Zakopane, 10 Kamieniec St., 34-500 Zakopane - Hospital Pharmacy, in the scope of Packet No. 1 - 98 assortment items according to the type and quantity list presented in point 4.10 of the said SWZ.2.The Ordering Party stipulates that the delivered subject matter of the contract must have all required current legal regulations, e.g. permits, certificates, attestations that prove the required marketing authorization and application in Poland for the intended use.3The goods must be marked by the manufacturer in such a way that both the product and the manufacturer can be identified.4 The Ordering Party stipulates that the offered goods must have an appropriately long period of validity allowing them to be used freely for at least one year, counting from the date of delivery.5The Ordering Party stipulates that the execution of supplies during the contract term will take place successively on the basis of partial orders placed by the Pharmacy Manager using electronic communication means.6. Each delivery batch must contain: product name, shelf life, batch number, name and address of the manufacturer/supplier. If the last day of delivery falls on a non-working day, the delivery shall be made on the first working day after the established time limit.7 Replacement of defective goods, goods not complying with the partial order, bad quality, delivered in wrong packaging shall be made within 48 hours of complaint submission at the expense and risk of the Contractor. If the last day of delivery resulting from the complaint falls on a non-working day, the delivery will take place on the first working day after the established time limit, within the hours mentioned in point 6.8 above. The Ordering Party informs that the quantities of particular pharmaceuticals (medicines) which are the subject of the present proceedings, given in point 4.10 of SWZ are the quantities calculated in the estimation manner and in the course of execution of the said contract may be subject to change, decrease by no more than 20%, depending on the actual needs of the Ordering Party.9The Ordering Party informs and explains that in scope of all Packets specified in point 4.10 SWZ, it agrees to change forms of oral forms, i.e. pricing of:1) instead of tablets - coated tablets or capsules or dragees,2) instead of coated tablets - tablets or capsules or dragees,3) instead of capsules - coated tablets or tablets or dragees,4) instead of irritants - tablets or coated tablets or capsules,5) instead of tablets, coated tablets or slow-release capsules - tablets or coated tablets or modified-release capsules, and vice versa.10Conditions of contract performance are included in the draft contract, which constitutes attachment No. 5 to the SWZ.11. The Ordering Party informs that the quantities of particular pharmaceuticals (medicines) which are the subject of the present proceedings, given in point 4.10 of SWZ are the quantities calculated in the estimation manner and in the course of execution of the said contract may be subject to change, decrease by no more than 20%, depending on the actual needs of the Ordering Party.9The Ordering Party informs and explains that in the scope of all Packages specified in item 4.10 SWZ, it agrees to change the forms of oral forms, i.e. pricing of:1) instead of tablets - coated tablets or capsules or dragees,2) instead of coated tablets - tablets or capsules or dragees,3) instead of capsules - coated tablets or tablets or dragees,4) instead of irritants - tablets or coated tablets or capsules,5) instead of tablets, coated tablets or slow-release capsules - tablets or coated tablets or modified-release capsules, and vice versa.10Conditions of contract performance are included in the draft contract, which constitutes attachment No. 5 to the SWZ.11. The Ordering Party informs that this procedure will be carried out according to the so-called "Reverse procedure", in accordance with the provisions of art. 139 of the Act - Public Procurement Law. Under this procedure the Contracting Authority shall first examine and evaluate tenders and then qualify the economic operator whose tender was awarded the highest score regarding no grounds for exclusion and meeting the conditions for participation in the procedure. The subject of this part of the contract is selection of a contractor who will provide the Ordering Party with supplies consisting of successive delivery by the contractor to the Dr Tytus Chałubiński County Hospital in Zakopane, 10 Kamieniec Street, 34-500 Zakopane - Hospital Pharmacy, of pharmaceuticals (medicines) within the scope of Packet No. 10 - 1 assortment item according to the type and quantity list presented in point 4.10 of the said SWZ.2.The Ordering Party stipulates that the delivered subject matter of the contract must have all required current legal regulations, e.g. permits, certificates, attestations proving the required marketing authorization and application in the territory of Poland in accordance with the intended use.3.The goods must be marked by the manufacturer in such a way that both the product and the manufacturer can be identified.4 The Ordering Party stipulates that the offered goods must have an appropriately long period of validity allowing them to be used freely for at least one year, counting from the date of delivery.5The Ordering Party stipulates that the execution of supplies during the contract term will take place successively on the basis of partial orders placed by the Pharmacy Manager using electronic communication means.6. Each delivery batch must contain: product name, shelf life, batch number, name and address of the manufacturer/supplier. If the last day of delivery falls on a non-working day, the delivery shall be made on the first working day after the established time limit.7 Replacement of defective goods, goods not complying with the partial order, bad quality goods, goods delivered in wrong packaging shall be made within 48 hours of complaint submission at the expense and risk of the Contractor. If the last day of delivery resulting from the complaint falls on a non-working day, the delivery will take place on the first working day after the established date, within the hours referred to in point 6.8 above.
