Description

The subject of the present order are consecutive supplies of veterinary medicines for large animals to the Faculty of Veterinary Medicine of the University of Warmia and Mazury in Olsztyn. 2. - The supplied subject of the contract will come from the current production, will have all the legally required approvals and certificates allowing them to be marketed in the territory of the Republic of Poland, which the contractor will be obliged to present if required by the relevant provisions of Polish law or requested by the contracting organizational unit.
The quantities of veterinary drugs specified in the description of the subject of the contract reflect the purchases made on the basis of previously concluded contracts. Due to the specificity of this public procurement i.e. the lack of possibility to foresee the quantitative range for this type of purchases, for the purpose of offer evaluation the ordering party accepted indicative quantities. In the course of performance of the contract, the Ordering Party reserves the right to place orders for quantities greater than those specified by the Ordering Party in individual items in the Description of the Object of Purchase/Price Form or not to purchase a given item at all, but declares that it will purchase at least 40% of the drugs specified in Appendix 1.The Contracting Authority allows submitting equivalent bids in accordance with art. 99 item 5 of the PPL, however the requirements and technical parameters defining the subject matter of the contract given by the Contracting Authority are minimal conditions that the Contracting Authority will expect to be met. The Contractor applying for an equivalent solution is obliged to prove that the solution offered by him meets the requirements specified by the Contracting Authority. In such a case, the contractor shall attach to the tender a list of equivalent solutions, together with their description or standards.3.RODO - the entire text can be found in the SWZ section (Protection of personal data collected by the contracting authority in the course of the procedure).1.The contracting authority declares that it meets the requirements set out in Regulation of the European Parliament and of the Council(EU) 2016/679 of 27 April 2016. on the protection of natural persons in relation to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the EU L 119 of 4 May 2016) - hereinafter: RODO - thus the personal data provided by the Contractor will be processed in accordance with RODO and in accordance with national regulations.2.The administrator of your personal data is the University of Warmia and Mazury in Olsztyn, ul. M.Oczapowskiego 2, 10- 957 Olsztyn, NIP 739-30-33-097.3.The Contractor's personal data will be processed on the basis of Art. 4.Recipients of personal data provided by the Contractor shall be persons or entities to whom the procedure documentation will be made available pursuant to art. 8 and art. 96 item 3 of the PPL Act and art. 6 of the Act of 6 September 2001 on the protection of personal data in public procurement procedures. 5. The Economic Operator's personal data contained in the protocol of the procedure will be kept for 4 years from the end of the award procedure. If the duration of the contract exceeds 4 years, the storage period shall include the whole contract duration.6. The information clause referred to in art. 13 par. 1 and 2 RODO is in appendix no. 2 to the SWZ (Terms of Reference).7. If the controller plans to process personal data for a purpose other than the purpose for which the personal data were collected (i.e. the purpose specified in point b above), the controller shall, prior to such further processing, inform the data subject of that other purpose and provide him/her with all other relevant information referred to in Article 13(2) of RODO.8.
