Description
You must be able to develop:
- a registration system linked to the patient portal platform (Malaffi) that enables patients to register with the primary care provider
- a communication and integration between primary and secondary care, including referral management, discharge letters and patient's choice of provider
- the primary care business case and communication strategy outlining the positive role and function of primary care for both patients and the health system
Primary care development represents a change in emphasis from secondary care to appropriate services that provide the initial point of entry to all health services.
