Description

The subject of the order is the delivery of other implants and a bone substitute to the hospital warehouse for 24 months.2The order has been divided into 6 parts/packages and includes: Package no.; package name: 1. plates for forearm bone anastomosis;2. block plates for end of radial bone anastomosis;3. plates for cot bones anastomosis;4. bone substitute;5. conical hip socket;6. bioblockwork nails for stabilization of minor cartilage injuries.
Due to the specificity of the subject of the contract and inability to predict the actual demand, the Ordering Party reserves the right to move the goods in the material and quantitative scope, with the reservation that the introduced changes cannot result in exceeding the financial obligation specified in the offer and the concluded contract. The Contractor may only offer products that have been introduced to the market in accordance with the provisions of the Act of 20 May 2010 on Medical Devices (Journal of Laws of 2017, item 211). The Contractor may offer an assortment from one and the same manufacturer within a given package. The Orderer does not allow to create a hybrid of implants from elements that have been produced by different manufacturers. A detailed description of the subject of the order can be found in the price form attached as an attachment No. 1 a to the ToRInfoIncumentsInformation on instrumentsIn case of assortment ordered on the basis of written orders using the Loaner Set method, it is required to deliver the instruments for each treatment. In case of assortment placed in deposit with the Orderer, it is required to equip the operating block with an instrumentarium for implantation, which will be lent for the duration of the contract.1The Contractor is obliged to carry out the order according to the terms and conditions described in the specimen agreement constituting Attachment No. 4 A (applies to implants in deposit) and 4B to the ToR (applies to implants supplied by the Loaner Set method).2 The Contractor is obliged to carry out the necessary training of the Orderer's personnel concerning the offered assortment. The Purchaser requires at least one training in this area. Detailed terms and conditions of training are included in the model contract which constitutes Annex 4A, 4B to the ToR. If the description of the subject matter of the contract specifies any trade mark, patent or origin - it should be assumed that the indicated patents, trade marks, origin specify technical, operational and utility parameters, which means that the Awarding Entity allows for submitting a tender in this part of the subject matter of the contract with equivalent technical, operational and utility parameters. At the same time, we would like to remind you that pursuant to Article 30(5) of the PPL Act, a Economic Operator who invokes solutions equivalent to those described by the Contracting Authority is obliged to prove that the subject matter of the contract offered by him meets the requirements specified by the Contracting Authority.
