Description

The subject of the contract is the delivery together with assembly of furniture for the needs of the Medical University of Gdańsk2. Technical, qualitative and quantitative requirements are specified in annexes 3 and 4 to the ToR.3The Contracting Authority may first evaluate the bids and then examine whether the economic operator whose bid has been assessed as the most advantageous is not subject to exclusion and fulfils the conditions for participation in the procedure (Article 24aa paragraph 1 of the Public Procurement Law).4 The bid must contain the following statements and documents: a) a completed bid form drawn up on the basis of the model attached as Appendix 1 to the ToR, b) a detailed description of the equipment offered (packages 1- 8), in accordance with Appendix 3 to the ToR - the document is to enable the bid to be evaluated, c) the statement of JEDZ referred to in Chapter V paragraph 1 of the ToR, c) the statement of JEDZ referred to in Chapter V paragraph 1 of the ToR. 1-3 of this ToR - attachment No. 2 to the ToR, d) if applicable - commitment of a third party signed with a qualified electronic signature, e) original of the guarantee/surety if the contractor makes a deposit in a form other than a cash deposit, f) if applicable - power of attorney referred to in Chapter VI, paragraph 2 of the ToR, f) if applicable - power of attorney referred to in Chapter VI, paragraph 2 of the ToR, g) if the contractor makes a deposit in a form other than a cash deposit. 1 point 4) ToR with a qualified electronic signature, g) Document confirming the appointment by Economic Operators jointly applying for the contract, a proxy to represent them in the procurement procedure or to represent them in the procurement procedure and to conclude a public procurement contract (applies only to Economic Operators jointly applying for the contract) - the document should be presented in the form provided for in Chapter VI point 1 point 4) ToR for a proxy.4In this procedure, the Contracting Authority demands that the Contractors pay a deposit in the amount of: 500 PLN.
