Description

Subject of the order is purchase and delivery of medical equipment for diagnosis and treatment of lung cancer for Szpital Specjalistyczny w Prabuty Sp. z o.o. (Specialist Hospital in Prabuty, Poland), whose detailed description, as well as description of the parts - packages, list of equipment, quantities and requirements are specified in Annexes 1 and 3 to the SWZ (Terms of Reference).2 The subject of the order is divided into 6 parts - packages.3 A detailed description of the subject of the order and the conditions of performance and delivery are regulated in the assortment-price form, which constitutes Annex No 3 to the SWZ (Terms of Reference), and a draft agreement, which constitutes Annex No 2 to the SWZ (Terms of Reference).
