Description

The subject matter of the contract is the purchase of the ultrasound and x-ray apparatus together with construction works for Hospital Copernicus PL Sp. z o.o.CPV 33100000-1 Medical equipment,CPV 71320000-7 Engineering services in design (for lot no. 4).The subject matter of the contract consists of 4 parts:- part no. 1: ultrasound machine type I and II,- part no. 2: ultrasound machine type III and IV,- part no. 3: bone and lung x-ray machine,- part no. 4: bone and lung x-ray machine with construction works.Detailed description of the subject of the contract determining the Purchaser's requirements, quantities, are included in appendix no. 1A to SWZ - Price Form and in appendix no. 1B to SWZ - Description of the subject of the contract (OPZ), which the Economic Operator encloses to the tender. By filling in the indicated positions the Economic Operator confirms that he meets the requirements specified by the Contracting Authority. The scope of construction works, being a part of the subject matter of the contract in part no 4, is specified in the draft contract.
The detailed required parameters of the device and the quantity are shown in appendix no. 1 B to SWZ (OPZ).The offered items of the contract have to be allowed for circulation and use in accordance with applicable law.The subject matter of the contract has to comply with the act of 20th May 2010 on medical devices (i.e. Dz.U.2020In case of assortment described by reference to standards, technical evaluations, technical specifications and technical reference systems mentioned in art. 101 par. 1 point 2 and par. 3 of the PPL Act, the Contracting Authority shall allow solutions equivalent to those described. Required warranty period: min. 24 months, For the criterion "Technical evaluation" the Contracting Authority shall evaluate offers on the basis of information provided in the Description of the subject matter of the contract - appendix 1B to the SWZ. The number of points to be scored in the criterion "Technical evaluation" shall be calculated according to the formula indicated in the SWZ.The equipment supplied under this order, in particular medical equipment, must be brand new and must consist of brand new components, as well as conform to the relevant standards applicable in Poland and be supplied without any defects. The equipment must be complete and ready to use after installation without any additional purchases and investments. Required warranty period: min. 36 months, For the criterion of "Technical evaluation" the Contracting Authority will evaluate the offers on the basis of information included in the Description of the subject matter of the contract - appendix 1B to the SWZ. The number of points to be scored in the criterion of "Technical evaluation" will be calculated on the basis of the formula indicated in the SWZ.The offered items of the contract must be brand new, complete, of a high standard both in terms of quality and functionality, and free from material, design and legal defects.The offered items of the contract must be allowed for marketing and use in accordance with the applicable provisions of law.The equipment supplied under this contract, in particular medical equipment, must be brand new and must consist of brand new subassemblies, as well as comply with the relevant standards applicable in Poland and be delivered without any defects. The equipment must be complete and ready for use after installation without any additional purchases or investments.
