The subject matter of the contract is the supply of rehabilitation and diagnostic device for early neurological rehabilitation with electromyography and ultrasound device for the needs of Copernicus Podmiot Leczniczy Sp. z o.o. in Gdańsk.Part 1: rehabilitation and diagnostic device for early neurological rehabilitation with electromyographyPart 2: ultrasound device with Duplex option Detailed description of the subject matter of the contract determining the Ordering Party's requirements and quantities are presented in Appendix no. 1A to SWZ - Price Form and in Appendix no. 1B to SWZ - Description of the subject matter of the contract (OPZ).
Detailed description of the subject matter of the contract determining the Ordering Party's requirements and quantities are included in Appendix 1A to SWZ - Price Form and in Appendix 1B to SWZ - Description of the subject matter of the contract (OPZ), which the Economic Operator encloses to the tender. By filling in the indicated positions the Economic Operator confirms that he meets the requirements defined by the Contracting Authority. The offered items of the contract must be complete, of a high standard both in terms of quality and functionality, as well as free from material, design and legal defects.The offered items of the contract must be allowed for circulation and use in accordance with the applicable provisions of law.The equipment supplied under this contract, in particular medical equipment, must be brand new and must consist of brand new subassemblies, as well as comply with the relevant standards applicable in Poland and be delivered without any defects. In all the provisions of SWZ and its appendices where the Contracting Authority refers to standards, approvals, technical specifications or reference systems, or where trademarks, parameters or sources of origin are indicated (names of manufacturers or equipment), pursuant to Article 99 par. 5 and Article 101 par. 4 of the PPL Act, the Contracting Authority shall allow equivalent solutions.
