The subject matter of the contract is purchase and delivery of specialist medical equipment for the existing hospital wards of Mazowiecki Szpital Specjalistyczny Sp. z o.o. in Radom. The scope of the contract also includes training of the Ordering Party's medical and technical staff, as well as warranty and service services related to the subject of the Delivery, specified in the draft provisions of the contract.The order consists of 6 parts: part 1 - Arthroscope column with set of instruments - 2 sets part 2 - Pelvic buckle - 1 set part 3 - Variable pressure mattresses - 20 pcs.part 4 - Transport trolley - 2 pcs.part 5 - Treatment trolley - 2 pcs.part 6 - Polysomnograph class IV with video surveillance - 1 pc.
The subject of the contract is the purchase and delivery of an arthroscopic column with a set of instruments - 2 sets with parameters specified in detail in Annex A to the SWZ. The scope of the contract also includes training of the Ordering Party's medical and technical staff and warranty and maintenance services related to the subject of delivery, as specified in the contract, For products not classified as medical devices and in accordance with European directives and the Law on Medical Devices are not covered by declarations of conformity and are not subject to any registration, the Purchaser requires a relevant statement. The subject matter of the contract is the purchase and delivery of a pelvic buckle - 1 set with parameters specified in detail in Annex B to the SWZ.The scope of the contract also includes training of the Ordering Party's medical and technical staff and warranty and maintenance services related to the subject matter of the delivery, as specified in the contract.The supplied equipment must be brand new, complete and approved for marketing and use in accordance with the Act of 20 May 2010 on medical devices (Journal of Laws of 2020, For products not classified as medical devices and in accordance with European directives and the Law on Medical Devices are not covered by declarations of conformity and are not subject to any registration, the Purchaser requires an appropriate statement. The subject matter of the contract is the purchase and delivery of 20 high-pressure adjustable mattresses with parameters specified in detail in Annex C to the SWZ (SWZ). The scope of the contract also includes training of the Ordering Party's medical and technical personnel as well as warranty and maintenance services related to the subject matter of the delivery, as specified in the contract.The supplied equipment must be brand new, complete and approved for marketing and use in accordance with the Act of 20 May 2010 on medical devices (Journal of Laws of 2020, For products not classified as medical devices and in accordance with European directives and the Law on Medical Devices are not covered by declarations of conformity and are not subject to any registration, the Purchaser requires a relevant statement. The subject matter of the contract is the purchase and delivery of treatment trolleys - 2 units, with parameters specified in detail in Annex E to the SWZ.The scope of the contract also includes training of the Ordering Party's medical and technical staff and warranty and maintenance services related to the subject matter of the delivery, as specified in the contract.The supplied equipment must be brand new, complete and approved for marketing and use in accordance with the Act of 20 May 2010 on medical devices (Journal of Laws of 2020, For products not classified as medical devices and in accordance with European directives and the Law on Medical Devices are not covered by declarations of conformity and are not subject to any registration, the Purchaser requires a relevant statement. The subject matter of the contract is the purchase and delivery of a class IV polysomnograph with video surveillance - 1 unit, with parameters specified in detail in Annex F to the SWZ.The scope of the contract also includes training of the Ordering Party's medical and technical staff and warranty and maintenance services related to the subject matter of the delivery, as specified in the contract.The supplied equipment must be brand new, complete and approved for marketing and use in accordance with the Act of 20 May 2010 on medical devices (Journal of Laws of 2020, For products not classified as medical devices and in accordance with European directives and the Law on Medical Devices are not covered by declarations of conformity and are not subject to any registration, the Purchaser requires an appropriate statement.
