Description

1) The subject matter of the contract is the delivery of medical equipment for "Poddębicki Centrum Zdrowia" Sp. z o.o. in Poddębice. (2) A detailed description of the subject matter of the contract including the technical parameters required by the Purchaser has been defined in the Price Form and the technical parameters of the medical equipment - Annex 2 to the SWZ. The subject matter of the contract has been defined in the Common Procurement Dictionary under the CPV code: 33100000-1, 33169500-7. 4. The equipment supplied must be brand new, not post-issue, non-regenerated, manufactured in 2021, free from physical and legal defects, complete and ready to use after installation, at no additional cost and have a user manual in Polish.

(2) A detailed description of the subject matter of the contract including the technical parameters required by the Ordering Party has been specified in the Price Form and the technical parameters of the medical equipment - Annex 2 to the SWZ. The subject matter of the contract has been defined in the Common Procurement Dictionary under the CPV code: 33100000-1, 33169500-7. 4. The equipment supplied must be brand new, not post-issue, non-regenerated, manufactured in 2021, free from physical and legal defects, complete and ready to use after installation, at no additional cost and have a user manual in Polish. The Ordering Party requires that the subject of the contract offered by the Ordering Party meets the requirements set out in the applicable European Union law, has been admitted to the market and has the legally required documents stating that it has been admitted to use in Poland, as well as meets other requirements (standards, parameters) set out by the Ordering Party in Appendix No. 2 to the SWZ. 6th The subject of the delivery must meet the requirements of the Polish standards transposing European harmonized standards or - in their absence - the European technical approvals, common technical specifications, Polish standards transposing European standards, Polish standards implementing international standards, Polish technical approvals. (7) The subject matter of the contract classified as a medical device/equipment for medical devices must be admitted to marketing and use in the territory of the Republic of Poland in accordance with the provisions of the Act of 20 May 2010 on medical devices and should be in possession of the documents admitting the products to marketing and use required under the aforementioned Act. (8) The Purchaser requires that the warranty period for the medical equipment is a minimum of 24 months, 36 months or a maximum of 48 months, counting from the date of signing the acceptance protocol without any defects. During the warranty period, the Contractor shall provide free of charge inspections. (9) The Purchaser requires payment to be spread over 5 equal instalments without additional costs. The payment term of each installment is 30 days, whereby the payment term of the first installment is calculated from the date of delivery to the Purchaser of a correctly issued VAT invoice. Form of payment transfer to the account indicated by the Economic Operator 10th The Economic Operator is obliged to execute the order on the terms and conditions described in the agreement template, constituting an attachment No. 5 to the SWZ. (11) The Contractor shall deliver the medical equipment within 31 days of signing the contract
