Description

The subject of the contract is the delivery of two (2) unused equipment manufactured in 2019 from the manufacturer's official distribution channel in Poland or the European Union, free of third-party rights, installation, training, commissioning, and all activities related to the proper functioning of the equipment for two (2) centres/direct customers: - one (1) accelerator with all necessary activities (including storage and insurance) for the needs of the Provincial Welfare Centre for Oncology and Traumatology in Warsaw (Wojewódzki Wielospecialistyczny Centrum Onkologii i Traumatologii im. M. Kopernika in Łódź, - one (1) accelerator with all necessary activities (including storage and insurance) for the needs of the West Pomeranian Cancer Center in Szczecin.1A detailed description of the subject of the contract (required parameters) must be attached to the offer form together with the "Detailed offer form" which constitutes Appendix No. 2 to the ToR.1.3. If the offered subject of the contract does not meet all the required parameters, the offer will be rejected without further evaluation.2Pursuant to § 13.1.3 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the Contracting Authority may request from the Economic Operator in the procurement procedure - Journal of Laws of 27.7.2016, item 1126, in connection with the Ordinance of the Minister of Enterprise and Technology of 16 October 2018 amending the Ordinance on the types of documents that the Contracting Authority may request from the Economic Operator in the procurement procedure - Dz.U. of 17 October 2018, item 1993, in order to confirm that the offered deliveries meet the requirements specified by the Ordering Party, the Contractor is obliged to confirm with a proper document, i.e. submit together with the offer form (subject matter conditions): 2.1.1. Declaration of conformity issued by the manufacturer for conformity with essential requirements for devices, in accordance with the requirements of Directive 98/79/EC or the regulations implementing these requirements into national legislation, 2.1.2. Detailed technical specification of the offered medical devices/apparatus including the equipment name, manufacturer's name, characteristics and certificates and/or certificates and/or original catalogues with photographs - folders (company information materials - FMI), issued by the manufacturer, in which should be marked all the parameters required by the Ordering Party and the parameters assessed in the "Detailed offer form "2.1.4. If there are no such parameters in the company information materials - FMI, the Contractor is obliged to confirm these data with a relevant statement signed by a person authorized to represent the Contractor. In the absence of a clear confirmation in the FMI or in the Economic Operator's statement of the value of the parameters offered, the Contracting Authority has the right to assess the parameter with a zero rating in case of any doubts. In case of doubt, the Ordering Party reserves the right to verify the parameters of the offered medical devices/apparatus on the basis of the original manufacturer's materials.2.2. The documents referred to above are submitted in the original in the form of an electronic document or in an electronic copy of a document or a statement certified for conformity with the original and accompanied by a qualified electronic signature.
