Description
The Wilhelmshaven Hospital with its department for radiotherapy is the only radio-oncological facility in the Wilhelmshaven area with a full range of treatments and thus provides care for approx. 76,500 inhabitants. At present, about 850 patients are treated annually, of which about 250 patients with benign diseases and about 600 as tumor patients, all of whom are treated at the existing Varian linear accelerator built in 2007. The facility will be installed in a newly constructed radiation protection bunker. The new electron linear accelerator is to be added to the existing system. The new facility to be procured is to cover all clinical issues of modern radiation therapy and enable high-quality patient care for the next 10 to 15 years even with increasing patient numbers. Further mandatory requirements for the new equipment to be procured are described in the specifications.
The Wilhelmshaven Clinic intends to expand its inventory by an electron linear accelerator for the department of radiation therapy, which will be installed in a newly built radiation protection bunker. With the new electron linear accelerator the existing system consisting of an electron linear accelerator "Clinac DHX", the oncology information system "Aria" as well as the radiation planning system "Eclipse" of the company Varian shall be supplemented. In order to be able to treat patients with 3D conformal and intensity-modulated radiation technology without replanning effort at both facilities, it has to be ensured that the compatibility of the beam properties of both electron linear accelerators is given. This applies above all to the energy matching to the existing Clinac. The specifications of the matching for all energy levels must be given. If deviations in matching are to be expected due to different system configurations (e.g. standing wave, travelling wave or modernized/new waveguides or energy switches), these must be specified. If matching is not possible due to different system design, the concept for a simple failure concept must be presented. System integration with the oncology information system is given priority for reasons of effectiveness, patient safety and data security. Compatibility, conformity and certification of the entire system according to MPG is a prerequisite The new system to be procured should cover all clinical issues of modern radiotherapy and enable high-quality patient care for the next 10 to 15 years, even with increasing patient numbers.In addition to standard techniques, the focus will be on image-guided radiotherapy with CT and MV imaging (IGRT) including Cone Beam CT, Intensity Modulated Radiotherapy (IMRT), Intensity Modulated Rotational Radiotherapy (VMAT), Surface Guided Radiotherapy (SGRT), Surface-guided RT), motion irradiation (rotation) as well as intra- and extracranial stereotactic treatments, motion management (gating) and electronic portal imaging detector (EPID) as well as portal dosimetry system for verification of intensity-modulated irradiation plans also for FFF modes.A failure concept for the existing Varian linear accelerator must be presented.the new accelerator must be redundant in terms of a failure concept and efficient patient management.the concept to be implemented must fully support the current and the future planned clinical spectrum of the department of radiotherapy and therefore requires the following specifications. This is not only about the procurement of a new linear accelerator, but also about the maintenance or expansion of a functioning overall system consisting of therapy, documentation and administration, hereinafter referred to as the operating procedure system (BAS).
