The subject of the order is: a. Delivery of reagent kits for determination of immunoglobulins and specific proteins by immunoturbidimetry as well as control material and calibrators in the assortment, estimated quantity and requirements specified in Annex 3 to the SWZ. b. Lease of analyzer with printer and additional equipment according to Annex No. 4 to the SWZ.2. Supply of the assortment indicated in item 1a will be realized on the basis of partial orders. Maximum delivery time: 14 working days from the date of placing the order. The ordered assortment shall be delivered by the Contractor at his own expense.

The assortment mentioned in point 1a has to meet the requirements specified in the order form. 3. The assortment mentioned in point 1a has to fulfill the requirements specified in attachment No 3 to SWZ. Failure to meet at least one of the requirements specified in Appendix 3 to the SWZ will result in rejection of the offer. 4. 4. The analyzer mentioned in point 1b must meet the parameters specified in Annex 4 to the SWZ. Failure to meet at least one of the parameters specified in appendix 4 to the SWZ will result in rejection of the offer.5. In addition, the Contractor shall be obliged to:- install the supplied analyzer at the direct user's site within 21 days from the date of signing the agreement;- provide training in the operation of the analyzer;- provide operating instructions in Polish: in printed form and in electronic form on CD/Pendrive for the analyzer and additional equipment (to be delivered with the delivery of the subject of the lease);- to provide with the delivery of the subject of the lease a copy of the certificate and declaration of conformity with the EC directive;- to provide maintenance service of the system, which is the subject of the lease by the Lessor, including inspections, maintenance, repairs together with the delivery of parts and consumables necessary for inspections, maintenance and repairs, as well as software updates; within the scope of the lease rent;- delivery with the delivery of the subject of the lease of the technical passport;- granting the guarantee for the offered popma for the whole lease period;- removal of the failure or insertion of the substitute device of the equivalent apparatus/equipment by the service of the Contractor in the time not longer than 3 working days from the moment of reporting the damage by the Ordering Party;- possibility of reporting the failure on working days from 8.Software updates throughout the duration of the contract;
