Description

Under this order, the Contractor shall be obliged to: 1) lease to the Ordering Party a scanner, laboratory cradle, pipette with tips and a computer program for electronic evaluation of allergy test results, hereinafter referred to as "equipment "1.1) the replaced equipment must be free from legal and physical defects and work on the ordered reagents,1 2) the equipment must be free of defects in law and in fact it must be free from any other physical defects.2) the requirements specified in Annex 3 to the ToR should be treated as minimum requirements set by the Awarding Entity for the offered equipment,1.3) the price of the offer shall include service at the Awarding Entity's seat and technical support under the conditions specified in the draft agreement,1.4) the price of the offer shall include training of the Awarding Entity's personnel in the use of the offered equipment.22.1) the offered reagents must be in a quantity securing the quantitative performance of all determinations listed in Annex No. 2 to the ToR,2.2) the offered reagents must have expiry dates guaranteeing the optimal use of the ordered products, i.e. minimum 6 months from the date of their delivery to the headquarters of the Purchaser - the offered reagents must meet all the requirements of the Act on Medical Devices of 20.5.2010. (Journal of Laws of 2019.175)2.3) the offered reagents must meet the essential requirements specified in the Regulation of the Minister of Health of 12.1.2011 on essential requirements and conformity assessment procedures for in vitro diagnostic medical devices (i.e. Journal of Laws of 2013, item 1127, as amended) and the annexes to the aforementioned Regulation, the Contracting Authority additionally requires the Contractor to: 1) fill in the assortment and price form attached as Appendix No. 2 to the Specification of Essential Terms of Order, failure to fill in all the fields will result in rejection of the offer. If the number of marks in the packaging is indivisible, it should be rounded up to a full package,2) fill in Annex 3 to the Specification of Important Order Conditions - List of technical and user parameters of allergy testing equipment, failure to fill in all fields (with particular reference to the column containing the description of offered parameters) will result in rejection of the offer,3) list in Annex 2 and calculate a sufficient amount of reagents, as well as other calibration, control and operational materials if necessary to perform allergy testing on the offered equipment. If at least 1 element is not taken into account or the quantity of assortment is underestimated in relation to the demand, the Economic Operator shall supply a given element or a missing quantity of goods at its own expense. Economic Operators should submit statements and documents in electronic form, with a qualified electronic signature, valid as at the date of submission of tenders, including a Declaration in the form of a Single European Procurement Document, hereinafter referred to as JOY, prepared in accordance with the standard form specified in the European Commission executive regulation issued pursuant to Article 59(2) of Directive 2014/24/EU and Article 80(3) of Directive 2014/25/EU. The information contained in the statement will constitute a preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the procedure.
