Description

The subject matter of this order is the supply of chemical reagents with accessories for the Municipal Hospital Complex in Chorzów in the quantity specified in the SAC constituting Annex No. 2 to this SWZ. The execution of the order for package no. 1, 2, 7, 13, 14 also includes the lease for a period of 36 months of laboratory equipment, i.e., depending on the package: semi-automatic (package 1) / analyzer (package 2, 13) / multidosing pipettes, centrifuge, microcard reader (package 7) / vending machine (package 14). The parameters of the equipment are specified in the "Description of the subject of the order" (hereinafter in its content: OPZ), which is attached as Appendix No. 3 to the SWZ.
3.1 Term of order execution: a) successive supplies of reagents and accessories:I. Package No. 1 - The Purchaser expects the execution of the order from the date of delivery, installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order: from the date of signing the acceptance protocol of the equipment (not earlier than 21.09.2022) - package 1 b) delivery, installation and commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place on the date specified by the Contractor in the Bid Form, with the Ordering Party specifying the maximum date, i.e. 35 days from the date of conclusion of the contract and in the next maximum 7 days training of personnel. Delay in the delivery, installation and commissioning and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7.1.c) of the Agreement, but the accrual of lease rent may begin no earlier than from the date of signing the final acceptance protocol of the semi-automatic (package 1).3.2. Delivery will be made in accordance with the current needs of the Ordering Party. Delivery date of reagents: Package 1 - a maximum of 5 working days (i.e., Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1 Order completion date: a) successive deliveries of reagents and accessories:I. Package No. 2 - The Ordering Party expects order completion from the date of delivery, installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order: from the date of signing the acceptance protocol of the equipment (not earlier than 26.08.2022) - package 2 b) the delivery, installation and commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place on the date specified by the Contractor in the Bid Form, with the Ordering Party specifying the maximum date, i.e. 35 days from the date of conclusion of the contract and in the next maximum 7 days training of personnel. Delay in delivery, installation and commissioning and commissioning of the leased equipment shall be subject to the contractual penalty referred to in §7.1(c) of the Agreement, but the accrual of lease rent may begin no earlier than from the date of signing the final acceptance protocol of the analyzer (package 2)3.2. Deliveries shall be made in accordance with the current needs of the Ordering Party. Delivery date of reagents:a) Package 2 - maximum 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 am to 2:00 pm) from the date of order notification, 3.1. Order completion date: a) successive deliveries of reagents and accessories: The ordering party expects the execution of the order for a period of 36 months: from 16.12.2022. - Package 33.2 Deliveries will be made in accordance with the current needs of the Purchaser. Delivery date of reagents:Package 3 - in accordance with the date declared in the offer form (maximum delivery date: 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1 Order completion date: a) consecutive delivery of reagents and accessories: The Ordering Party expects to execute the order for a period of 36 months: from 20.10.2022 - package 43.2. Deliveries will be made in accordance with the current needs of the Ordering Party. Delivery date of reagents:Package 4 - in accordance with the date declared in the offer form (maximum delivery date: 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1 Order completion date: a) consecutive supply of reagents and accessories: The ordering party expects the execution of the order for a period of 36 months: from 25.10.2022. - Package 53.2 Deliveries will be made in accordance with the current needs of the Purchaser. Delivery date of reagents:Package 5 - in accordance with the date declared in the offer form (maximum delivery date: 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1 Order completion date: a) successive delivery of reagents and accessories: The ordering party expects the execution of the order for a period of 36 months: from 19.11.2022. - package 63.2.Delivery will be made in accordance with the current needs of the Purchaser. Delivery date of reagents:Package 6 - in accordance with the date declared in the offer form (maximum delivery date: 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1 Order completion date: a) successive deliveries of reagents and accessories:I. Package No. 7 - The Purchaser expects order completion from the date of delivery, installation of laboratory equipment for a period of 36 months. Purchaser expects the execution of the contract: from the date of signing the protocol of acceptance of equipment (not earlier than 26.10.2022). - package 7b) the delivery, installation and commissioning and commissioning of the leased laboratory equipment for a period of 36 months will take place on the date specified by the Contractor in the Bid Form, with the Employer specifying the maximum date, i.e. 35 days from the date of conclusion of the contract and in the next maximum 7 days staff training. Delay in delivery, installation and commissioning and commissioning of the leased equipment shall be subject to the contractual penalty referred to in §7.1(c) of the Agreement, however, accrual of lease rent may begin no earlier than from the date of signing the final acceptance protocol of the multidosing pipettes, centrifuge, microcard reader (package 7).3.2 Deliveries will be made in accordance with the current needs of the Purchaser. Delivery date of reagents:Package 7 - in accordance with the date declared in the bid form (maximum delivery date: 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1 Order completion date: consecutive deliveries of reagents and accessories: Package No. 8 - The Purchaser expects to execute the order for a period of 12 months from the date of conclusion of the contract.3.2. Delivery will be made in accordance with the current needs of the Purchaser. Delivery date of reagents: Package 8 - in accordance with the date declared in the offer form (maximum delivery date: 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1 Order completion date: consecutive deliveries of reagents and accessories: Package No. 9 - The Ordering Party expects to execute the order for a period of 24 months from 20.10.2022.3.2. The execution of supplies will take place according to the current needs of the Ordering Party. Delivery date of reagents:Package 9 - in accordance with the date declared in the bid form (maximum delivery date: 10 working days (i.e. Monday to Friday except public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification. 3.1 Order completion date: a) consecutive deliveries of reagents and accessories: The Ordering Party expects to execute the order for a period of 36 months: from the date of conclusion of the contract - package 103.2. Deliveries will be made in accordance with the current needs of the Ordering Party. Delivery date of reagents:Package 10 - in accordance with the date declared in the offer form (maximum delivery date: 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1 Order completion date: a) consecutive delivery of reagents and accessories: The ordering party expects the execution of the order for a period of 36 months: from 10.11.2022. - package 113.2 Deliveries will be made in accordance with the current needs of the Purchaser. Delivery date of reagents: in accordance with the date declared in the offer form (maximum delivery date: 5 working days (i.e. Monday to Friday except public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification. 3.1 Order completion date: a) consecutive deliveries of reagents and accessories: The Ordering Party expects to execute the order for a period of 36 months: from the date of conclusion of the contract - package 123.2. Deliveries will be made in accordance with the current needs of the Ordering Party. Delivery date of reagents: in accordance with the date declared in the offer form (maximum delivery date: 5 working days (i.e. Monday to Friday except public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification. 3.1 Order completion date: a) consecutive deliveries of reagents and accessories: The Ordering Party expects to execute the order from the date of delivery, installation of laboratory equipment for a period of 36 months. The Ordering Party expects the execution of the order: from the date of signing the acceptance protocol of the equipmentb) delivery, installation and commissioning and commissioning of the leased laboratory equipment for a period of 36 months will be carried out within the timeframe specified by the Contractor in the Bid Form, with the Ordering Party specifying the maximum timeframe, i.e. 35 days from the date of conclusion of the contract and in another maximum 7 days staff training. Delay in delivery, installation and commissioning and commissioning of the leased equipment shall be subject to the contractual penalty referred to in §7.1(c) of the Agreement, but the accrual of lease rent may begin no earlier than the date of signing the final acceptance protocol of the analyzer (package 13)3.2. Deliveries will be made in accordance with the current needs of the Ordering Party. Reagents delivery date: maximum 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 a.m. to 2:00 p.m.) from the date of order notification, 3.1. Order completion date: a) consecutive delivery of reagents and accessories: The Ordering Party expects to execute the order from the date of delivery, installation of laboratory equipment for a period of 36 months. The Employer expects the execution of the order: from the date of signing the acceptance protocol of the equipment.b) the delivery, installation and commissioning and commissioning of the leased laboratory equipment for a period of 36 months will be carried out within the period specified by the Contractor in the Bid Form, with the Employer specifying a maximum period of 35 days from the date of conclusion of the contract and in the next maximum 7 days staff training. Delay in delivery, installation and commissioning and commissioning of the leased equipment shall be subject to the contractual penalty referred to in § 7.1.c) of the Agreement, but the accrual of lease rent may begin no earlier than from the date of signing the final acceptance protocol of the machine (package 14).3.2. Delivery will be made in accordance with the current needs of the Ordering Party. Delivery date of reagents: a maximum of 5 working days (i.e. from Monday to Friday except for public holidays from 8:00 am to 2:00 pm) from the date of order notification,
