Description

The subject matter of the contract is the supply of reagents for immunochemistry together with lease of an immunochemical analyzer for the needs of the Center for Pulmonology and Thoracic Surgery in Bystra, in quantities specified in detail in appendix no 2 and with the technical parameters specified in appendix no 3 to the tender, under conditions specified in the draft agreement constituting appendix no 6 to the tender. The scope of the contract also includes installation and start-up of the supplied analyzer within 14 working days from the date of conclusion of the contract, training of selected employees of the Ordering Party in the use and proper operation of the system, as well as providing free of charge ongoing maintenance, including periodic inspections/technical tests of the system, as well as authorized service of the supplied system. The expiration date of the supplied reagents shall not be less than 6 months.

The offered products (reagents, analyzer) which are medical devices must have current documents allowing marketing and use and meet the requirements specified by law, in particular the Law of 07.04.2022 on medical devices ((i.e. Journal of Laws of 2022, item 974.) along with other implementing regulations to the cited law, the Regulation of the Minister of Health of 17.02.2016. on Essential Requirements and Conformity Assessment Procedures for Medical Devices, and must be CE marked and have a valid CE Declaration of Conformity, if required by lawThe offered reagents that are hazardous preparations must have a Material Safety Data Sheet for a hazardous preparation as defined in the Act of February 25, 2011 on chemical substances and their mixtures (i.e. Journal of Laws 2020, item 2289, as amended), which the Contractor will be obliged to provide with the first delivery of reagents.
