DZP/11/20 The subject of the order is the supply of reagents, calibrators, substrates, tests and consumables together with the lease of diagnostic analysers for the Department of Laboratory and Microbiological Diagnostics with the Blood Bank and the Department of Pathology of the Specialist Hospital in Kościerzyna, divided into 8 tasks.The Contracting Authority does not provide for supplementary orders referred to in art. 67 par. 1 point 6 and 7 of the PPL.
The lease of the immune analyser together with the supply of reagents, control materials and consumables. The lease of an automatic system for blood and body fluids cultures with an attachment for staining microscopic preparations with the Gama method, together with the supply of appropriate substrates and reagents. Lease of an automatic system for identification of microorganisms and determination of their drug-sensitivity together with delivery of reagents and tests. The highest-rated Contractor shall be requested to submit the following declarations or documents confirming that the offered deliveries meet the requirements specified by the Purchaser:1. instruction manual in Polish language;2. technical manual of equipment;3. CE declaration;4. ISO 9001 and 13485 quality certificate for medical devices;5. current safety data sheets required (according to the regulation Min. Zdr. of 13.11.2007) for the offered reagents classified as hazardous substances within the meaning of the regulations on chemical substances and preparations (regulation Min. Zdr. of 2.9.2003 on the list of hazardous substances together with qualification and marking - Journal of Laws 03.199.1948);6. 6. test methodology in Polish for each of the offered tests;7. list - list of identifiable microorganisms for each of the offered tests;8. list of all types of identification and drug-sensitivity tests being in the Contractor's offer as of the date of submitting the offers, together with the manufacturer's catalogue numbers;9. sample of the test cassette (test panel) in the amount of 1 piece; 10. 10. a list of detected mechanisms of Gram resistance (+) positive bacteria confirmed by the manufacturer;11. a list of detected mechanisms of Gram resistance (-) negative bacteria confirmed by the manufacturer;12. identification methodology and determination of drug-sensitivity from possible growth media (basic, selective, chromogenic) in Polish;13. manufacturer's documentation confirming detection of drug resistance mechanisms. Delivery of biochemical reagents, serological reagents and protein electrophoresis kits together with lease of necessary equipment. Lease of a back-up analyzer for the Integra 400 plus by ROCHE with supply of compatible reagents, control materials, calibrators and consumables Lease of two coagulology analyzers with successive supply of reagents, calibrators, Control and consumables Rental of two critical parameters analysers (main and replacement with successive supply of reagents, calibrators, controls and consumables Rental of an immunohistochemical analyser with successive supply of reagents
