Description

The subject of the contract is successive supplies of consumable materials for procedures performed in continuous techniques, citrate dialysis, heparin hemofiltration, plasmapheresis, whose assortment and estimated quantities are specified in the price form attached as Appendix No. 1 together with the lease of equipment for procedures performed in continuous techniques - 14 items. The offered subject of the contract must be admitted to trading and use in Poland in accordance with the Act on Medical Devices (Journal of Laws of 2019, item 175) - refers to items 1-3, 6-12, 14 of the price form (attachment No. 1). In such a case, the brands or names indicated by the Ordering Party define the class of the product being the subject of the order and serve to determine the standard, and do not indicate a specific product or a specific manufacturer. The Economic Operator who refers to equivalent solutions described by the Contracting Authority is obliged to demonstrate that the offered subject of the contract meets the requirements specified by the Contracting Authority (legal basis of Article 30 paragraph 5 of the Act) by submitting a technical description and providing the name of the product and the manufacturer. The Contracting Authority does not allow partial bids. The Contracting Authority does not allow variants to be submitted (art. 36 sec. 2 item 4 of the Public Procurement Law). The Contracting Authority does not provide for the award of contracts referred to in art. 67 sec. 1 item 4 of the Public Procurement Law. The Contracting Authority does not provide for the conclusion of a framework agreement (Article 36(2)(2)(2) of the PPL). The Contracting Authority does not provide for the settlement in foreign currencies (Article 36(2)(6) of the PPL). The Contracting Authority does not provide for the holding of an electronic auction (Article 36(2)(7) of the PPL). The Contracting Authority does not provide for the reimbursement of costs of participation in the procedure (Article 36(2)(7) of the PPL). Contract completion date: - supply, installation and commissioning of apparatus within 6 weeks from the date of contract conclusion, - lease of apparatus within 48 months from the date of contract conclusion, - successive deliveries of consumables for procedures performed in continuous techniques - within 48 months from the date of contract conclusion: 125 048,00 PLN
