Description

The subject of the order is the civil liability insurance, property and vehicle insurance of the Independent Public Voivodship Hospital in Szczecin, which includes 2 tasks.2 The Contractor is obliged to execute the order under the terms and conditions described in the draft agreement, constituting Appendix No. 4A and/or 4B to SWZ, and in the description of the subject of the order, constituting Appendix No. 1 to SWZ.
Civil liability insurance of the Independent Public Voivodeship Complex Hospital in Szczecin, constituting Task 1, includes: 1) mandatory civil liability insurance of the entity providing medical treatment,2) civil liability insurance on account of its business activity and property.2 Detailed description of the subject of the contract constitutes attachment No. 1 to the SWZ.3The Contracting Authority contemplates the possibility of awarding contracts referred to in Article 214 par. 1 item 7 of the PPL within a period of 3 years from the date of awarding the basic contract.4 The contracts referred to in item 3 above will consist in the repetition of services conforming to the services constituting the subject matter of this contract and services similar to them.5 The material scope of the services constituting the subject matter of the contracts referred to in item 4 above will not exceed PLN 101 550.40 net.6.The contracts referred to in item 3 above shall be awarded following a separate procedure for the award of a public contract under a sole-source contract.7.The contracts mentioned in point 3 above will be awarded in case of a need to extend the insurance period due to justified circumstances, in particular due to a prolonged tender procedure which makes it impossible to award a contract for a subsequent period in due time under an open tender procedure or a basic tender procedure, or due to difficulties in preparation of a tender procedure due to events beyond the Employer's control, in particular due to force majeure (e.g. epidemics/pandemics).8 The place of performance of the contracts mentioned in point 3 above shall be the territory of the Republic of Poland.9The Contracting Authority does not reserve the obligation for the Contractor to personally execute key tasks related to the subject matter of the contract. The Economic Operator may entrust elements (parts) of the subject matter of the contract to subcontractors. If the Economic Operator intends to carry out the subject matter of the contract with the participation of subcontractors, he is obliged to indicate in his tender the parts of the contract (material areas) whose performance he intends to entrust to subcontractors and to name possible subcontractors, if known. Such indication shall be included in the Tender. If no subcontractor is indicated in the Tender, the Economic Operator will be able to introduce a subcontractor only under the conditions specified in the contract.10. The Contracting Authority requires that the Economic Operator or its subcontractor employs, on the basis of a contract of employment, people who perform activities included in the subject matter of the contract which consist in carrying out administrative activities related to issuing/annexing insurance documents, if the performance of these activities consists in carrying out work in the manner specified in art. 22 § 1 of the Act of 26 June 1974. - Labour Code (uniform text: Journal of Laws of 2020, item 1320 as amended). The insurance of property and motor insurance of the Independent Public Voivodeship Complex Hospital in Szczecin, constituting Task No. 2, includes: 1) all-risk insurance of property,2) all-risk insurance of electronic equipment,3) breakdown insurance of machines and devices,4) motor insurance: OC, NNW, AC, ASS.2. The detailed description of the subject matter of the contract constitutes attachment No. 1 to the SWZ.3. The Contracting Authority anticipates the possibility of awarding contracts referred to in Article 214 (1) (7) of the PPL within a period of 3 years from the date of awarding the basic contract.4. The contracts referred to in item 3 above will consist in the repetition of services conforming to the services constituting the subject matter of this contract and services similar to them.5.The material scope of the services constituting the object of the contracts referred to in item 4 above shall not exceed the value of PLN 113,579.40 net.6.
