The subject matter of the order is the manufacture and delivery of an anti-theft backpack according to all requirements defined in the Detailed Description of the Object of Procurement (hereinafter: SHOPZ/SOPZ or OPZ), which is an annex to the Specification of Terms and Conditions of Procurement (hereinafter: SWZ) within the framework of the health policy programme of the Minister of Health titled "Ensuring self-sufficiency of the Republic of Poland in blood and its components for the years 2021-2026 "The order has to be completed in its entirety according to the requirements defined in SWZ, PPU and SOPZ.
The subject of the order is the execution and delivery of anti-theft backpack in the amount of 8000 pieces.2Pursuant to art. 105 and 106 of the PPL Act, in order to confirm compliance of the offered supplies with the requirements, features or criteria specified in the description of the subject matter of the order or criteria for evaluation of tenders or requirements connected with performance of the order, the Contracting Authority demands from the contractors to present:2.1.1. One (1) item of the subject matter of the order in the form of the pattern, meeting all the requirements specified by the Contracting Authority in SOPZ without marking. The specimen must be delivered to the registered office of the Department of Public Procurement at the Ministry of Health located at 155 Jerozolimskie Avenue, 02-326 Warsaw, Monday through Friday, 09:00-15:00 until the submission deadline. After evaluation, the specimens will be returned to the contractor upon request pursuant to Article 77 of the PPL Act. 2.1.2. Product data sheets in Polish language confirming that the product is made of the indicated material.2.1.3. Contractor's and/or manufacturer's statement that the supplied item will not exceed 700 grams in weight.2.2. - i.e. in the Regulation of the President of the Council of Ministers of 30 December 2020 on the manner of preparing and submitting information and technical requirements for electronic documents and means of electronic communication in a public procurement procedure or a competition.2.3. The Contracting Authority, pursuant to art. 65 item 1 point 4) of the PPL. waives the use of electronic means of communication with respect to the specimen of the subject matter of the contract. The manner of its submission has been described above.2.4. The Contracting Authority shall reject the tender pursuant to art. 226 item 1 point. 2 letter c. 2.5. The Contracting Authority pursuant to art. 107 item 2 of the PPL Act 2.6. Documents in a foreign language shall be submitted together with a translation into Polish.2.7. Pursuant to Article 107(1) of the PPL Act the Contractor shall submit the above-mentioned means of evidence together with the tender and shall make every effort to ensure that the said documents are complete and contain all the necessary information to enable the Contracting Authority to verify their correctness.
