Description

The subject of the order is the execution and delivery of promotional materials (gadgets) as part of the loyalty program for Blood Donors called "Every drop is valuable".The purchase is carried out in the framework of the implementation of the health policy program of the Minister of Health called "Ensuring self-sufficiency of the Republic of Poland in blood and its components for 2021-2026" in the specific objective: "Maintaining a constant number of informed donors of blood and its components", "Increasing public awareness of honorary blood donation", "Increasing awareness of healthy nutrition among blood donors".
The subject matter of the contract is execution and delivery of a game set consisting of 5 dice and 54 cards in an edition of 17 000 pieces (sets).2 Pursuant to Article 257 of the PPL Act. The Contracting Authority may invalidate the award procedure if the public funds that the Contracting Authority intended to allocate to finance the whole or part of the contract were not awarded to him3. Description of the subject matter of the contract (hereinafter referred to as OPZ), containing requirements and conditions for performance of the contract constitutes the appendix to the Terms of Reference.4. Subject matter of the evidence:Pursuant to provisions of art. 106 of the PPL Act, in order to confirm compliance of the supplies offered with the requirements, features or criteria specified in the description of the subject matter of the contract or evaluation criteria for tenders, or requirements related to performance of the contract, the Contracting Authority demands from Economic Operators to present the following documents:4.1..2 A statement of the Economic Operator on the basis of the paper weight of playing cards.4.3 Pursuant to provisions of art. 107 item 1 of the PPL Act, the Economic Operator shall submit the above mentioned means of evidence together with the offer, however if it is necessary to provide a specimen of the offered subject of the contract, the Contracting Authority shall require it to be submitted to the seat of the entity conducting the proceedings - i.e. the Department of Public Procurement at the Ministry of Health, Al. Jerozolimskie 155, 02-326 Warsaw, during its working hours (Monday to Friday between 08:00 and 16:00) - before the deadline for submission of offers. The statement mentioned in item 4.2. should be attached to the offer on the Platform, affixed with a qualified electronic signature by an authorized person.4.4.The Contracting Authority, pursuant to provisions of art. 107 item 2 of PPL, shall allow the above mentioned means of evidence to be supplemented once.5. When selecting the most advantageous offer the Contracting Authority reserves the right to reduce the purchase, however not more than by 20% of the entire subject matter of the contract in case the amount allocated for its realization is exceeded. The subject matter of the contract is manufacturing and delivery of red T-shirt in edition of 15 500 pieces.2. The Contracting Authority may invalidate the award procedure if the public funds that the Contracting Authority intended to allocate to finance the whole or part of the contract were not awarded to him3. Description of the subject matter of the contract (hereinafter referred to as OPZ), containing requirements and conditions for performance of the contract constitutes the appendix to the Terms of Reference.4. Subject matter of the evidence:Pursuant to provisions of art. 106 of the PPL Act, in order to confirm compliance of the supplies offered with the requirements, features or criteria specified in the description of the subject matter of the contract or evaluation criteria for tenders, or requirements related to performance of the contract, the Contracting Authority demands from Economic Operators to present the following documents:4.1..2. statement of the Economic Operator regarding the weight and type of material of the T-shirt.4.3 Pursuant to provisions of art. 107 item 1 of the PPL Act, the Economic Operator shall submit the above mentioned means of evidence together with the offer, however if it is necessary to provide a specimen of the offered subject of the contract, the Contracting Authority shall require that it be submitted to the seat of the entity conducting the proceedings - i.e. the Department of Public Procurement at the Ministry of Health, Al. Jerozolimskie 155, 02-326 Warszawa, during its working hours (Monday to Friday from 08:00 a.m. to 04:00 p.m.) - before the deadline for submission of offers. The statement mentioned in item 4.2. should be attached to the offer on the Platform, affixed with a qualified electronic signature by an authorized person.4.4.The Contracting Authority, pursuant to provisions of art. 107 item 2 of PPL, shall allow the above mentioned means of evidence to be supplemented once.5. When selecting the most advantageous offer the Contracting Authority reserves the right to reduce the purchase, however not more than by 20% of the entire subject matter of the contract in case the amount allocated for its realization is exceeded. 1) The subject matter of the contract is manufacture and delivery of a bamboo breakfast box in a quantity of 14 500 pieces.2) Pursuant to art. 257 of the PPL Act the Contracting Authority may declare the proceedings invalid. The Contracting Authority may invalidate the award procedure if the public funds that the Contracting Authority intended to allocate to finance the whole or part of the contract were not awarded to him3. Description of the subject matter of the contract (hereinafter referred to as OPZ), containing requirements and conditions for performance of the contract constitutes the appendix to the Terms of Reference.4. Subject matter of the evidence:Pursuant to provisions of art. 106 of the PPL Act, in order to confirm compliance of the supplies offered with the requirements, features or criteria specified in the description of the subject matter of the contract or evaluation criteria for tenders, or requirements related to performance of the contract, the Contracting Authority demands from the Economic Operators to present the following documents:4.1.4.2.Declaration of the Economic Operator regarding the capacity of the offered breakfast box.4.3 Pursuant to provisions of art. 107 item 1 of the PPL Act, the Economic Operator shall submit the above mentioned evidence together with the offer; however, if the Contracting Authority requires the submission of the sample of the offered subject matter of the contract with the offer, the Contracting Authority shall require it to be submitted to the registered office of the entity conducting the proceedings, i.e. the Department of Public Procurement at the Ministry of Health, Al. Jerozolimskie 155, 02-326 Warsaw, during its working hours (Monday to Friday between 8:00 a.m. and 4:00 p.m.) - before the deadline for submission of offers. The statement mentioned in item 4.2. should be attached to the offer on the Platform, affixed with a qualified electronic signature by an authorized person.4.4.The Contracting Authority, pursuant to provisions of art. 107 item 2 of PPL, shall allow the above mentioned means of evidence to be supplemented once.5. When selecting the most advantageous offer the Contracting Authority reserves the right to reduce the purchase, however not more than by 20% of the entire subject matter of the contract in case the amount allocated for its realization is exceeded. The subject matter of the contract is manufacture and delivery of a sports towel in a sack in the number of 13 000 pieces.2 Pursuant to provisions of art. 257 of the PPL Act. The Contracting Authority may invalidate the award procedure if the public funds that the Contracting Authority intended to allocate to finance the whole or part of the contract were not awarded to him3. Description of the subject matter of the contract (hereinafter referred to as OPZ), containing requirements and conditions for performance of the contract constitutes the appendix to the Terms of Reference.4. Jerozolimskie 155, 02-326 Warszawa, during its working hours (Monday to Friday from 08:00 a.m. to 04:00 p.m.) - before the deadline for submission of tenders. The statement mentioned in item 4.2. shall be attached to the offer on the Platform, affixed with a qualified electronic signature by an authorized person.4.4.The Contracting Authority, pursuant to provisions of art. 107 item 2 of PPL, shall allow the above mentioned means of evidence to be supplemented once.5. When selecting the most advantageous offer the Contracting Authority reserves the right to reduce the purchase, however not more than by 20% of the entire subject matter of the contract in case the amount allocated for its realization is exceeded. The subject matter of the contract is manufacturing and delivery of an ecological mug with an ear, 10 000 pieces.2. The Contracting Authority may invalidate the award procedure if the public funds that the Contracting Authority intended to allocate to financing the whole or part of the contract were not awarded to him3. Description of the subject matter of the contract (hereinafter referred to as OPZ), containing requirements and conditions for performance of the contract constitutes the appendix to the Terms of Reference.4. Subject matter of evidence:Pursuant to provisions of art. 106 of the PPL Act, in order to confirm compliance of the supplies offered with the requirements, features or criteria specified in the description of the subject matter of the contract or evaluation criteria for tenders, or requirements related to performance of the contract, the Contracting Authority demands from the Economic Operators to present the following documents:4.1..2 A statement by the Economic Operator regarding the capacity of the cup, the types of materials it is made of and the percentage of each material content.4.3 Pursuant to provisions of art. 107 item 1 of the PPL Act, the Economic Operator shall submit the above mentioned means of evidence together with the offer, however if it is necessary to provide a specimen of the offered subject of the contract, the Contracting Authority shall require it to be submitted to the registered office of the entity conducting the proceedings - i.e. the Department of Public Procurement at the Ministry of Health, Al. Jerozolimskie 155, 02-326 Warszawa, during its working hours (Monday to Friday from 08:00 a.m. to 04:00 p.m.) - before the deadline for submission of offers. The statement mentioned in item 4.2. shall be attached to the offer on the Platform, affixed with a qualified electronic signature by an authorized person.4.4.The Contracting Authority, pursuant to provisions of art. 107 item 2 of PPL, shall allow the above mentioned means of evidence to be supplemented once.5. When selecting the most advantageous offer the Contracting Authority reserves the right to reduce the purchase, however not more than by 20% of the entire subject matter of the contract in case the amount allocated for its realization is exceeded. The subject matter of the contract is the manufacture and delivery of a folding umbrella with a 3-part metal cane in the edition of 8 000 pieces.2 Pursuant to art. 257 of the PPL Act the Contracting Authority may invalidate the proceedings. The Contracting Authority may invalidate the award procedure if the public funds that the Contracting Authority intended to allocate to finance the whole or part of the contract were not awarded to him3. Description of the subject matter of the contract (hereinafter: OPZ) containing requirements and conditions for performance of the contract constitutes the appendix to the Terms of Reference.4 Subject matter of the evidence:Pursuant to provisions of art. 106 of the PPL Act, in order to confirm compliance of the supplies offered with the requirements, features or criteria specified in the description of the subject matter of the contract or evaluation criteria for tenders, or requirements related to performance of the contract, the Contracting Authority demands from Economic Operators to present the following documents:4.1..2.Pursuant to provisions of art. 107 item 1 of the PPL Act, the Economic Operator shall submit the above-mentioned evidence together with the tender, however if it is necessary to submit the model of the offered subject matter of the contract, the Contracting Authority shall require that it be submitted to the office of the person in charge of the procedure - i.e. the Department of Public Procurement at the Ministry of Health, Al. Jerozolimskie 155, 02-326 Warszawa, during its working hours (Monday to Friday between 08:00 and 16:00) - before the deadline for submission of tenders.4.3.The Contracting Authority pursuant to art. 107 item 2 of the PPL provides for the possibility to supplement the above mentioned evidence once.5.When selecting the most advantageous offer the Contracting Authority reserves the right to reduce the purchase, however not by more than 20% of the total subject matter of the contract in case the amount allocated for its execution is exceeded.
