Description
Together with IKK BB, AOK Nordost is planning the approval procedure for integrated care services for geriatric patients in need of nursing care for the period of funding from the Innovation Fund. This is to be established in outpatient and inpatient nursing facilities (hereinafter referred to as applicants) and the pharmacies providing care. A significantly improved risk communication between the participants is to be achieved by means of geriatric qualification and implementation of quality requirements across sectors and occupational groups. The nursing staff, pharmacists and doctors involved receive a dual in-company training with university parts of practical geriatrics in order to ensure the technical expertise on site. The software provision as well as the geriatric qualification is carried out by the pharmaceutical consortium partner specified in the funding decision.Through risk screening, increased risk-benefit assessment, targeted therapy monitoring and risk communication, the following objectives are to be achieved:- implementation of drug risk management systems,- avoidance/reduction of adverse drug events (UAE),- permanent and sustainable reduction of drug-induced risk situations,- the future contractual partners provide the insured persons on the basis of the contract and are responsible for the integration of the necessary additional service providers in accordance with Point III.1..3. 1.d) or III.1.3. 2.d).the service providers must meet the personal, technical and quality requirements described in the application for participation (including forms).the remuneration is lump-sum. The contracts will not start before 14.6.2019. The open procedure form will only be used because there is no admission procedure form. Contracts which do not involve a selection decision among the suitable candidates are not subject to public procurement law. The use of this form does not imply submission to procurement regulations.
