Description

Task 1 - consisting in the provision of the organisation's service (i.e. planning and carrying out) of health promotion and colorectal cancer prevention in the form of individual information and educational meetings at the Copernicus PL Sp. z o.o. location, ul. Nowe Ogrody 1-6 in Gdańsk in the number of 70 hours, by 31.12.2020. The number of contractors to be commissioned. 1 - 1.2. With regard to the services provided, the Contractor shall be obliged to: a) select and recruit people from the target group to participate in the project; b) plan and conduct individual information and educational meetings; c) provide promotional materials provided by the Contracting Authority; d) provide the Contracting Authority with correctly completed declarations of participation in the project.The Contractor shall be obliged to appoint at least 1 person, hereinafter referred to as an educator (a doctor or a nurse or a midwife or a graduate of a public health university), who shall be trained by the Contracting Authority in the prevention of colorectal cancer. Task 2 - consisting in the provision of the organization's service (i.e. planning and carrying out) of health promotion and colorectal cancer prevention in the form of individual information and educational meetings in the location of Copernicus PL Sp. z o.o., ul. Nowe Ogrody 1-6 in Gdańsk in the amount of 50 hours, by 31.12.2020. The number of contractors to be commissioned. 2 - 1.With regard to the services provided, the Contractor shall be obliged to: a) select and recruit people from the target group to participate in the project; b) plan and conduct individual information and educational meetings; c) provide promotional materials provided by the Contracting Authority; d) provide the Contracting Authority with correctly completed declarations of participation in the project.The Contractor shall be obliged to appoint at least 1 person, hereinafter referred to as an educator (a doctor or a nurse or a midwife or a graduate of a public health university), who shall be trained by the Contracting Authority in the prevention of colorectal cancer. Task 3 - consisting in realization of the service of organization (i.e. planning and carrying out) of health promotion and prevention of colorectal cancer in the form of individual information and educational meetings on the territory of Słupsk County in total amount of 915 hours, i.e. 102 hours per month, until 31.12.2020. The number of contractors to be commissioned. 3 - 1The Contractor shall be obliged to: a) select and recruit people from the target group to participate in the project; b) plan and conduct individual information and educational meetings; c) provide promotional materials provided by the Contracting Authority; d) provide the Contracting Authority with correctly completed declarations of participation in the project.The Contractor shall be obliged to appoint at least 1 person, hereinafter referred to as an educator (a doctor or a nurse or a midwife or a graduate of a public health university), who shall be trained by the Contracting Authority in the prevention of colorectal cancer. Task 4 - consisting in the implementation of the service of organization (i.e. planning and carrying out) of health promotion and prevention of colorectal cancer in the form of individual information and educational meetings on the territory of Starogard County in the amount of 305 hours, i.e. approximately 34 hours per month, by 31.12.2020. The number of Contractors foreseen for Task 4 - 1 In the scope of provided services the Contractor shall be obliged, among others, to a) selecting and recruiting people from the target group to participate in the project; b) planning and conducting individual information and educational meetings; c) handing over promotional materials provided by the Contracting Authority; d) providing the Contracting Authority with correctly filled in declarations of participation in the project by the project participantsThe Contractor shall be obliged to appoint from among the persons referred to the service, at least 1 person, hereinafter referred to as an educator (a doctor or a nurse or midwife or a graduate of a public health faculty), who will be trained by the Contracting Authority in colorectal cancer prevention.
