The subject matter of the contract is delivery and installation of equipment in the ongoing investment under the name: "Construction of new KMPSP in Elbląg and adjacent JRG 1 in Elbląg at 19 Łęczycka Street - STAGE I".
The scope of Part 1 of the contract includes: - delivery of furniture and equipment, - delivery and placement, - assembly, - installation and commissioning, if required, - provision of service during the warranty period for the equipment requiring serviceFor a detailed description of the scope of the contract please see the SUPPLY PRICE attached as Appendix 1a to the SWZ.The Contracting Authority hereby informs that he does not intend to award a contract referred to in art. 214 par. 1 item 8 of the PPL Act.c.d. the entries from section II.2.14):The file (original guarantee or surety, in electronic form) should be submitted together with the tender by means of the Tender Submission Form or the application available at https://platformazakupowa.pl/transakcja/493230.c.d. records from section VI.3):4) Certificate or other document issued by a proper field organizational unit of the Social Insurance Institution (ZUS) or a proper regional branch or proper field unit of the Agricultural Social Insurance Fund confirming that the Economic Operator is not in arrears with payment of social and health insurance premiums, within the scope of art. 109 item 1 point 1 of the PPL Act, issued by the National Court Register. 1 point 1 of the PPL Act, issued not earlier than 3 months before submitting the document, and in case of arrears in payment of social or health insurance premiums with a certificate or other document the Contracting Authority demands submission of documents confirming that before the deadline for filing the tender the Economic Operator paid the due social or health insurance premiums with interests or fines or made a binding agreement on payment of such dues;5) extract or information from the National Court Register or Central Register and Information on Business Activity, within the scope of art. 109 par. 6) Economic Operator's statement confirming that the information contained in the declaration referred to in art. 125 item 1 of the PPL Act is up to date, regarding the grounds for exclusion from the proceedings indicated by the Contracting Authority and referred to in the following (c) Art. 108(1)(5) of the PPL Act, on entering into an agreement with other Economic Operators which is intended to distort competition,d) Art. 108(1)(6) of the PPL Act,e) Art. 109(1)(1) of the PPL Act, on violation of obligations relating to payment of local taxes and fees referred to in the Local Taxes and Fees Act of 12 January 1991 (Journal of Laws of 2019, item 1170).If the Economic Operator has his seat or place of residence outside the territory of the Republic of Poland, instead of documents mentioned in Section VI.3) item 4. points 1), 3)-5) of the notice - shall submit the documents specified in § 4 of the Regulation of the Minister of Development, Labour and Technology of 23 December 2020 on subjective evidence and other documents or statements which may be required by the Contracting Authority from the Economic Operator (Journal of Laws 2020 item 2415).For Parts 1, 2 and 3 of the contract:The Contracting Authority provides for the possibility to invalidate the award procedure if the public funds that the Contracting Authority intended to allocate to finance the whole or part of the contract have not been awarded to him.
