Description

I. The subject of the contract is the supply of electricity within the meaning of the Act of 10 April 1997. - Energy Law (i.e. Journal of Laws of 2019, item 755, as amended), to 4 entities holding PPE (electricity collection points), included in the following list: - buildings of the Medical University of Białystok in tariffs: C11- 3 PPE, C12a-1 PPE, C23- 6 PPE and G12w-3 PPE,- buildings of the University Clinical Hospital in Bialystok, in tariff group: B23 - 2 PPE; C22b - 2 PPE,- buildings of the University Children's Clinical Hospital in Białystok in tariff group B23 - 2 PPE,- buildings of the Independent Public Health Care Facility of the Ministry of the Interior and Administration in tariff group B23 - 2 PPE.A detailed description of the subject matter of the contract, the obligations of the contractor and the contracting authority is included in Appendix No. 6 to the ToR - a specimen agreement (general agreement with energy sales agreement).II. Contractors who are not subject to exclusion due to lack of grounds for exclusion due to failure to meet the conditions referred to in Article 24, section 1, items 12-23 of the Act may apply for the award of the contract. The Contracting Authority does not provide for the possibility of exclusion of the contractor pursuant to Article 24 paragraph 5 of Act III. An economic operator which relies on the capacities of other entities must prove to the contracting authority that, when performing the contract, it will have at its disposal the necessary resources of those entities, in particular by producing an undertaking by those entities to place at its disposal the necessary resources for the performance of the contract. The Contracting Authority requires the contractor to indicate the parts of the contract which it intends to entrust to subcontractors and the subcontractors' names. If the change or resignation from the subcontractor concerns the entity to whose resources the contractor referred, pursuant to the principles set out in Article 22a (1), In order to prove that the conditions for participation in the procedure are met, the contractor is obliged to prove to the Contracting Authority that the proposed other subcontractor or contractor meets them independently to an extent not less than the subcontractor to whose resources the contractor referred during the procurement procedure.IV. Submission of an offer1. The Economic Operator, according to the model specified by the contracting authority, prepares a tender, which it will submit in the public procurement procedure. The ToR with its attachments is placed on the website of the ordering party www.umb.edu.pl2.Wykonawca submits an offer/application to participate in the procedure, hereinafter referred to as the "application" by means of a form to submit, change, withdraw an offer or an application available on the ePUAP and also available on the miniPortal. The public key necessary to encrypt the bid by the Economic Operator is available to the Economic Operators on the miniPortal. In the offer/application form, the Economic Operator is obliged to provide the address of the ePUAP box where the correspondence related to the proceedings will be conducted.3 The offer/application should be prepared in the Polish language, preserving its electronic form in the following data format: pdf (recommended) and signed with a qualified electronic signature. The way of submitting an offer/application, including encryption of the offer, has been described in the rules of using the miniPortal. The offer/application must be submitted in original.
