Description

(1) The subject of the contract is the provision of early development support services for children within the framework of the "For Life" comprehensive support program for families. Early development support services will be provided in the form of individual classes with children aged from birth until they start school. One hour of early development support services shall be considered a full clock hour (60 minutes). 2. The subject of the contract is divided into 8 parts according to the estimated number of hours (h) of service:- cz1 estimated number of hours falls within the range of 400 - 500 h- cz2 estimated number of hours falls within the range of 150 - 200 h- cz3 estimated number of hours falls within the range of 100 -150 h- cz4 estimated number of hours falls within the range of 100 - 140 h- cz5 estimated number of hours falls within the range of 500 - 600 h- cz6 estimated number of hours falls within the range of 150 - 200 h- cz7 estimated number of hours falls within the range of 150-200 h- cz8 estimated number of hours falls within the range of 150-200h.
1 The subject of the contract is to provide early development support services for children within the framework of the comprehensive support program for families "For life". Early development support services will be provided in the form of individual classes with children aged from birth until they start school. One hour of early development support services is considered to be a full clock hour (60 minutes). 2 The estimated number of hours is in the range of 400 - 500 h3. The responsibilities of the specialist providing individual pedagogical therapy for children include:- having been appointed as a member of the team of specialists for a particular child, establishing the direction and schedule of activities undertaken in the field of early support and family support,- establishing cooperation with the facility where the child is included in other therapeutic activities to ensure consistency of interactions,- developing and implementing with the child and his family an individual program of early development support in the part intended for the pedagogue,- organizing meetings and consultations for the parents of the child, legal guardians of the child, actual guardians of the child,- assessing the progress and difficulties in the child's functioning, including the identification and elimination of barriers and limitations in the environment that hinder the child's activity and participation in social life,- analyzing the effectiveness of the assistance provided on the basis of the program to the child and his family,- making changes to the program in accordance with the current needs of the child and his family,- planning further activities in the field of early support.4Obligation to employ on the basis of a labor contract. The Employer requires that the Contractor or Subcontractor, throughout the performance of the subject matter of the contract, employ on the basis of an employment contract within the meaning of the provisions of the Act of June 26, 1974. - Labor Code of the persons performing the activities indicated in point.3.The full-time equivalent must correspond to the time necessary for the employees to perform all the activities covered by the service of providing early development support.The above condition will be fulfilled by hiring new employees on a contract of employment or appointing employees already employed by the Contractor to perform the contract. NOTE: the requirement does not apply to contractors who are natural persons who will perform the subject of the contract personally.
