Description

The subject matter of the contract is the supply of dressing materials to the Voivodship Hospital Emergency Station in Białystok, specified in detail in attachment no. 2 to the SWZ (Price Form). The order has been divided into parts: part 1 - miscellaneous dressings, part 2 - disinfectant swabs, part 3 - hydrogel dressings. The quantities given in the price form are estimated/predicted, and may be reduced over the course of the contract depending on the customer's current needs. During the term of the contract the Ordering Party guarantees an order on the level of 60 % of the assortment listed in appendix no 2 to SWZ - Price Form.
The subject of the contract is successive delivery of dressing materials to WSPR (Voivodship Emergency Station) in Białystok, specified in detail in appendix 2 to SWZ - Price Form. The quantities given in the price form are estimated/forecasted and may be reduced in the course of the contract depending on the Ordering Party's current needs. During the term of the contract the Ordering Party guarantees an order at the level of 60% of the assortment listed in appendix no. 2 to SWZ - Price Form. The Contractor shall be obliged to submit a statement that he possesses the CE declarations and certificates required by law, confirming the possibility of marketing the offered dressing materials in the territory of the Republic of Poland. At the same time, the Contractor undertakes to provide the Ordering Party with the above-mentioned documents upon each request for inspection.
