Description

The subject matter of the contract is the supply of laboratory reagents: - task 1 - COVID-19 diagnostic tests; - detailed description of the subject matter of the contract is provided in Annex 2b to SWZ (Terms of Reference).
3.3 The Ordering Party's requirements regarding the subject matter of the contract: 1) the medical product offered must have attestations, certificates and be registered in accordance with the law requiring attestations, certificates and registration; 2) the medical product offered must be placed on the market and used in accordance with the requirements of the Act of 20 May 2010 on medical devices (Journal of Laws of 2020, If the offered medical device does not require registration with the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor shall be obliged to perform at his own expense and within his own scope all activities necessary to meet all legal requirements related to marketing and use of the product in the territory of the Republic of Poland. The Contractor shall be obliged to perform such activities within the time limits resulting from the applicable regulations and provide the Ordering Party with the relevant documents confirming the performance of such activities.20Information on intended awards referred to in Article 214.1.7 and 214.1.8 if the Contracting Authority envisages awarding such awards.The Contracting Authority does not envisage awarding contracts referred to in Article 214.1.7 of the PPL Act.The Contracting Authority does not envisage awarding contracts referred to in Article 214.1.8 of the PPL Act.Description of the criteria which the Contracting Authority will use in selecting tenders, together with the weighting of those criteria and the method of tender evaluation.13During the offer evaluation the following criteria will be awarded points for each of the evaluated bids, according to the following rules: - price criterion - weighting: 60%,- deadline for execution of a partial contract - weighting: 40%.
