Description
The subject matter of the contract is successive delivery of single-use medical devices to Szpital Specjalistyczny w Prabuty Sp. z o.o. (Specialist Hospital in Prabuty, Poland), whose detailed description, as well as description of the parts - packages, list of products, estimated quantities and requirements are specified in Annex 3 to the SWZ (Terms of Reference).2 The subject matter of the contract is divided into 79 parts - packages.3 The Purchaser allows submitting partial offers. Detailed description of the subject matter of the contract and conditions of execution and delivery is regulated in the assortment-price form enclosed as appendix 3 to the SWZ (SWZ) and the draft agreement enclosed as appendix 2 to the SWZ (SWZ).
