Description

1.1. purchase of a tomograph;1.2. delivery with transport insurance;1.3. disassembly and disposal of an old apparatus;1.4. execution of a construction and technological design for renovation and adaptation of rooms;1.5. execution of construction works related to adaptation of rooms intended for installation of the tomograph;1.6. installation, commissioning and commissioning of the tomograph for clinical use;1.7. instructional training at the Employer's seat;2.The technical requirements (limits and evaluated) of the tomograph are specified in Annex 1 to the ToR. The Contractor is obliged to fill in all items of Annex 1 in accordance with the offered parameters. In the absence of data concerning the limit conditions (Annex 1), the Contracting Authority shall consider that the required parameter is not present in the offered configuration of the offered apparatus. The subject of the contract covers the development of a construction-technological design and execution of construction works in the following areas: preparation of construction-technological documentation in the construction and construction industry, sanitary, electrical and teletechnical in accordance with the technical and operating documentation of the new tomograph and the execution of repair-adaptation works in accordance with the construction-technological documentation. The Contractor is obliged to agree the construction and technological design with the Ordering Party and the Supplier of the tomograph. The Contractor is to attach to the construction and technological documentation an inventory of the rooms and a material and financial schedule for the execution and settlement of the works performed. The detailed scope of the subject matter of the contract is specified in the Functional and Operational Programme - attached as Appendix No. 2 to this ToR. It is allowed to start renovation and adaptation works during the design works in consultation with the Supervision Inspector, user and supplier of the tomograph.4The main subject of the contract must meet the requirements specified in the Act of 20 May 2010 on medical devices, in particular be marked with the CE mark, and if the conformity assessment was carried out under the supervision of a notified body, the CE mark must be accompanied by the identification number of the competent notified body.5 The minimum required warranty period for the main subject of the contract - 36 months from the date of signing the final protocol of acceptance of the contract subject. The Ordering Party requires that the Contractor, during the warranty period, perform free of charge the necessary technical inspections and maintenance of the equipment with the frequency recommended by the manufacturer.Minimum warranty period for the performed design and construction works - 36 months from the date of protocol acceptance of works.6The delivery and installation of the tomograph shall take place up to 21 weeks after the date of conclusion of the contract; 3) Clinical commissioning of the tomograph no later than 14 days after the date of completion of the installation of the tomograph; 4) Training for medical, engineering and technical personnel within a period to be agreed with the Ordering Party.
