Description

The subject matter of the contract is: supply of working clothes and personal protection equipment, divided into 7 (seven) parts. A detailed description of the subject matter of the contract is specified in annexes no: 1, 1.1. - 1.7., 2, 2.1, and no. 5 and 5.1 to SWZ, i.e.: Bid Form, Assortment and Price Specifications, Description of the subject matter of the contract and Draft provisions of the contract. The Contracting Authority shall accept solutions equivalent to standards, approvals, technical specifications and reference systems referred to in art. 99 and art. 101 of the PPL Act. The Contracting Authority shall be obliged to prove that the offered supplies meet requirements specified by the Contracting Authority. The Contracting Authority shall inform: if required evidence is not submitted or is incomplete, the Contracting Authority shall call for its submission or supplementation (according to chapter V SWZ.
Delivery of working clothing for Uniformed Services. A detailed description of the subject matter of the contract is given in appendixes no: 1, 1.1. - 1.7., 2, 2.1, and No. 5 and 5.1 to the SWZ, i.e.: Bid Form, Assortment and Price Specifications, Description of the subject matter of the contract and Draft provisions of the contract. In order to confirm the compliance of the offered deliveries with the requirements specified in the description of the subject matter of the contract, the Contracting Authority requires the following subject matter evidence to be submitted with the offer:1) a current certificate, confirming the implementation of a quality management system compliant with the requirements of ISO 9001:2015 (PN-EN ISO 9001:2015) for the production and/or marketing of workwear and PPE,2) declarations of conformity confirming that the subject matter of the contract meets the specified requirements, issued on the basis of the Regulation of the European Parliament and of the Council (EU) 2016/425 of 9 March 2016. on personal protective equipment and repealing Council Directive 89/689/EEC. The declarations of conformity referred to above should be drawn up in accordance with the Act of 13 April 2016 on conformity assessment and market surveillance systems.3) photographs, prospectuses and/or catalogues with descriptions in Polish. The subject matter of the order should be clearly presented in the prospectus and/or catalogue and/or on a photograph, in colour, making it possible to compare and check the description of the offered subject matter of the order with appendix nos. 2 and 2.1 to the SWZ describing the subject matter of the order in detail. At the same time the proposed designs of the subject of the order should be marked (e.g. with a highlighter) in the prospectus and/or catalogue and/or on the photograph - this applies to all the offered items. 4) Samples with dimensions min. 4) Samples, measuring min. 15x15 cm, of the base materials from which the objects of the order will be made, confirming the fulfilment of the parameters (in particular: colours, shade, material composition, in which the names of the textile fibres and their percentage and fabric weight are given). The samples should be clearly described, giving the possibility to compare and check the description of the offered items with appendix no. 2.1 describing in detail the items of the order - applies to offered items from part II items 1-4, 6-7, 10-14 and 17-18 (table) - appendix no. 1.2. to SWZ5) Specimens of the offered items confirming the fulfilment of the parameters (in particular: colours, composition, fulfilment of the standards) - specimens in the form of single items (one glove from the pair) should be clearly described, giving the possibility to compare and check the description of the offered subject of the contract with appendix no 2 and 2.1 describing in detail the subject of the contract - refers to the offered items from part I items 8-27 (table) - appendix no 1.The descriptions of the samples and specimens referred to above should contain the following information: a) name, address and trademark of the manufacturer; b) country of the manufacturer; c) raw material composition, stating the names of textile fibres and their percentage and fabric weight - if required; d) CE mark - if required; e) number of appropriate EN standard - if required; f) marking with the symbol of appropriate protection category - if required.The CONTRACTING Authority waives the requirement to use electronic means of communication due to the necessity to present samples and specimens of the offered items, which cannot be submitted by means of electronic communication. In the case of samples and specimens that are tangible in nature, the only possibility is to make them physically available (submit). They shall be prepared and provided with a procedure number and a handwritten signature and shall be submitted via a postal operator within the meaning of the Act of 23 November 2012 - Postal Law (Journal of Laws of 2020, item 1041), personally or through a messenger at the registered office of the CONTRACTING Authority (public office) in a separate envelope no later than 25.10.2021 until 09.00. Delivery of protective clothing for the Uniformed Services. A detailed description of the subject matter of the contract is specified in: appendixes no: 1, 1.1. - 1.7., 2, 2.1, and No. 5 and 5.1 to SWZ i.e.: Bid Form, Assortment and Price Specifications, Description of the subject matter of the contract and Draft provisions of the contract. The subject matter of the contract should be new, unused, complete and free from defects in materials and design, of very good quality, carefully manufactured from materials with stabilized shrinkage, resistant to destruction, ready for use, consistent with the description of the assortment and packed and marked in a manner indicated in Annex 1.1 to 1.7 SWZ describing in detail the individual items of the contract. Delivery of gloves and protective clothing for the OPBMR Service. Detailed description of the subject matter of the contract is specified in annexes no: 1, 1.1. - 1.7., 2, 2.1, and No. 5 and 5.1 to SWZ i.e.: Bid Form, Assortment and Price Specifications, Description of the subject matter of the contract and Draft provisions of the contract. The Contracting Authority shall accept solutions equivalent to standards, approvals, technical specifications and reference systems referred to in art. 99 and art. 101 of the PPL Act. Supply of warning tapes for the OPBMR Service. Detailed description of the subject matter of the contract is specified in annexes no: 1, 1.1. - 1.7., 2, 2.1, and No. 5 and 5.1 to SWZ i.e.: Bid Form, Assortment and Price Specifications, Description of the subject matter of the contract and Draft provisions of the contract. The Contracting Authority shall accept solutions equivalent to standards, approvals, technical specifications and reference systems referred to in art. 99 and art. 101 of the PPL Act. The Contracting Authority shall be obliged to prove that the offered supplies meet requirements specified by the Contracting Authority.
